Genesis HealthCare, a leading provider of healthcare services, is seeking an LPN MDS Coordinator to join its team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process through the completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process following completion guidelines.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• LPN in good standing with current state license.
• Prior experience in hospital, long-term care, or healthcare-related facilities.
• Knowledge of rehabilitative nursing, medical practices, and laws governing long-term care.
• Computer skills including Windows applications.
• AccuMed experience preferred.
