RN Care Coordinator, Ambulatory Care

Job Description

We are looking for a compassionate and skilled RN Care Coordinator to join our Ambulatory Care team at Denver Health. As an RN Care Coordinator, you will be responsible for coordinating patient care by collaborating with multidisciplinary teams and patients/families to ensure quality outcomes within care coordination parameters.

Responsibilities

• Facilitate and coordinate care for an assigned group of patients through patient-centered goals and plans.
• Work with healthcare teams to ensure timely provision of services and safe transitions in care.
• Advocate for patient benefits and address potential coverage barriers.
• Provide care management and utilize community resources for patient support.

Requirements

• Associate's Degree in Nursing and RN license in Colorado.
• Minimum of 3 years clinical experience required.
• Proficiency in case management principles and healthcare coordination.
• Strong communication skills and ability to work in diverse settings.
