Job Description

We are looking for a dedicated Home Care LPN to join ChristianaCare's HomeHealth team. As an experienced Licensed Practical Nurse, you will provide skilled nursing care and treatment to geriatric patients in the community. You will be responsible for implementing care plans, participating in discharge planning, and ensuring patient-centered care in collaboration with our supportive leadership team.

Responsibilities

• Provide skilled nursing care and prescribed treatments to geriatric patients.
• Implement patient-centered care plans in line with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
