Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN) to join Genesis Health, a leading provider of healthcare services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process, ensuring high-quality patient care and nursing center reimbursement. You will be responsible for completing items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) in compliance with regulations.

Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans based on resident needs and regulatory requirements.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare facility and knowledge of nursing practices and regulations.
• Computer skills, including proficiency with Windows applications.
• AccuMed experience preferred.
• Proficiency in RAI/PPS processes and experience in completing MDSs and Care Plans.
• Current BLS/CPR certificate.
