Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN) to join Genesis HealthCare as an LPN MDS Coordinator. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in accordance with guidelines.
• Contribute to developing individualized resident Care Plans, addressing resident needs and expected outcomes to comply with Federal and State regulations.
• Train new interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare-related facility.
• Familiarity with general, rehabilitative, and restorative nursing practices, medical procedures, and healthcare regulations.
• Proficiency in Windows applications and experience with AccuMed preferred.
• Previous MDS Coordinator experience desirable.
• Current BLS/CPR certificate required.
