Position: Licensed Practical Nurse (LPN) - MDS Coordinator

Key Responsibilities:

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Complete MDS and Care Area Assessments (CAAs) for appropriate patient care and reimbursement.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements:

• LPN license in good standing.
• Prior experience in healthcare facilities.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Computer proficiency in Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• BLS/CPR certification required.
