Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. You will have the opportunity to provide skilled nursing care and treatment to geriatric patients in the community, working closely with our supportive leadership team and utilizing our state-of-the-art Electronic Health Record system. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
