Licensed Practical Nurse MDS Coordinator

Job Description

Genesis Health is seeking a Licensed Practical Nurse (LPN) MDS Coordinator to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and reimbursement.

Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to the interdisciplinary team.

Requirements

• LPN license in good standing.
• Prior experience in a healthcare facility.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Computer skills, including proficiency in Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience preferred.
• BLS/CPR certification required.
