Job Description

We are seeking a dedicated LPN MDS Coordinator to join Genesis Health, a leading healthcare provider dedicated to delivering high-quality care and compassionate services. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs).

Position Highlights

• Direct the interdisciplinary RAI process and ensure compliance with guidelines for MDS completion and transmission.
• Contribute to developing individualized resident Care Plans, meeting Federal and State regulations.
• Facilitate training on clinical content and electronic completion of MDS items for new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare facility.
• Familiarity with nursing practices and regulations.
• Proficiency in computer applications with AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
• BLS/CPR certification required.
