LPN MDS Coordinator

Genesis HealthCare is seeking a dedicated LPN MDS Coordinator to join our team and participate in the Resident Assessment Process to ensure appropriate patient care and nursing center reimbursement. As an LPN in good standing, you will be responsible for completing items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) while adhering to regulatory guidelines and standards.

Position Highlights

• Direct the interdisciplinary RAI process for resident assessment.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train team members on clinical content and electronic MDS completion.
