Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a skilled and dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join Genesis HealthCare. As a vital member of our team, you will be responsible for participating in the Resident Assessment Process by completing key items on the Minimum Data Set to ensure optimal patient care and appropriate reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument process, ensuring compliance with guidelines for MDS completion and transmission.
• Contribute to the development of personalized resident Care Plans, addressing individual needs and projected outcomes.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in healthcare settings such as hospitals or long-term care facilities.
• Familiarity with general nursing practices and regulations governing long-term care.
• Proficiency in computer applications, especially Windows; AccuMed experience is a plus.
• Previous experience as an MDS Coordinator preferred.
• Ability to maintain BLS/CPR certification.
