Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a talented Licensed Practical Nurse to join our team as a Minimal Data Set Coordinator at Genesis Health. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring accurate completion and electronic transmission of the MDS.
• Contribute to developing personalized resident Care Plans according to Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare setting.
• Familiarity with general, rehabilitative, and restorative nursing practices and regulatory guidelines.
• Computer proficiency, especially in windows applications.
• AccuMed experience preferred.
• Demonstrated proficiency in completing MDSs and Care Plans.
• Current BLS/CPR certification.
