Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a skilled and dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join Genesis Health's team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and accurate nursing center reimbursement.

Position Highlights

• Lead and oversee the Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for MDS completion and electronic transmission.
• Contribute to the development of personalized resident Care Plans, focusing on resident needs and anticipated outcomes according to Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license in good standing.
• Prior experience in hospital, long-term care, or healthcare settings.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Knowledge of laws, regulations, and guidelines in long-term care.
• Computer skills including Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• Current BLS/CPR certification required.
