Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join Genesis Health, a leading provider of healthcare services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing items on the Minimum Data Set (MDS) and Care Area Assessments to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring compliance and accurate completion of the MDS.
• Develop individualized resident Care Plans in accordance with Federal and State regulations.
• Train team members on clinical content and electronic MDS completion.

Requirements

• LPN in good standing with current state licensure.
• Prior experience in hospital, long-term care, or healthcare facilities.
• Familiarity with nursing practices, laws, and regulations.
• Proficient in Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience desirable.
• Current BLS/CPR certification required.
