Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a qualified Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process through the completion of essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process following completion guidelines.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train new team members on clinical content and MDS completion.

Requirements

• Current LPN license in good standing.
• Prior experience in hospital or long-term care settings required.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills including Windows applications.
• AccuMed experience preferred.
• Prior experience as an MDS Coordinator advantageous.
• Proficiency in RAI/PPS processes.
