Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is looking for a talented Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process through the completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and proper nursing center reimbursement.

Position Highlights

• Lead the nursing center interdisciplinary Resident Assessment Instrument (RAI) process in compliance with regulations.
• Develop individualized resident Care Plans, identifying resident needs and expected outcomes.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Valid LPN license.
• Prior experience in a healthcare setting.
• Familiarity with nursing practices and regulations.
• Proficiency in RAI/PPS processes.
• Computer skills, including Windows applications.
