Minimal Data Set Coordinator - LPN

Job Description

Genesis Health is seeking a motivated Minimal Data Set Coordinator - LPN to join our team. As an LPN MDS Coordinator, you will be responsible for participating in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and related Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the nursing center interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans following Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Prior experience in healthcare facilities.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills, including Windows applications.
• AccuMed experience preferred.
• Experience in MDS Coordinator role is a plus.
• Current BLS/CPR certificate.
