Nurse-LPN MDS Coordinator

Job Description

We are looking for a dedicated Nurse-LPN MDS Coordinator to join Genesis Healthcare, a leading healthcare services provider. In this role, you will play a key part in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring accurate completion and electronic transmission of the MDS.
• Contribute to the development of personalized resident Care Plans following Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license.
• Prior experience in a healthcare setting.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer applications.
• AccuMed experience preferred.
• Knowledge of completing MDSs and Care Plans.
• Competent in RAI/PPS processes.
• Current BLS/CPR certification.
