Responsibilities

• Conduct home visit assessments of referred patients to evaluate medical conditions and functional capabilities.
• Provide direct patient care and develop rehabilitation plans.
• Proficiently document patient records using EMR systems.
• Maintain CPR certification.

Requirements

• Graduation from an accredited Physical Therapy (PT) program.
• Valid Physical Therapy license for New York.
• Experience in conducting PT assessments and developing treatment plans.
• Familiarity with EMR documentation.
