Job Description

We are looking for a compassionate and dedicated Licensed Practical Nurse (LPN/LVN) to join Aveanna Healthcare. As an LPN/LVN, you will be responsible for providing skilled nursing care to patients under the supervision of a Registered Nurse, following individualized care plans and upholding professional standards.

Position Overview

The Licensed Practical Nurse (LPN/LVN) is tasked with delivering high-quality patient care, assessing patient needs, documenting care activities, and maintaining compliance with healthcare regulations. This role involves collaborating with supervisors, physicians, and other healthcare professionals to ensure optimal patient outcomes.

Key Responsibilities

• Provide and coordinate patient care as per physician orders.
• Assess patient condition, report changes, and implement appropriate interventions.
• Document all care activities accurately and participate in care planning.
• Educate patients and families on self-care, disease management, and prevention.
• Maintain infection control measures and ensure patient safety.

Requirements

• Graduate of an accredited nursing school.
• Current, unrestricted state license as an LPN/LVN.
• Current CPR certification.
• Proficiency in clinical assessments and nursing documentation.

Preferred Qualifications

• Minimum of six months experience in a clinical care setting.
• Background in home health care.

Skills and Abilities

• Detail-oriented with excellent time management skills.
• Strong problem-solving and communication abilities.
• Ability to manage multiple tasks efficiently.

Physical Requirements

• Proficient in English language.
• Able to lift up to 50 pounds and assist with patient movements.
• Capable of responding to emergency situations.
