Job Description

We are seeking a dedicated Registered Nurse (RN) Field Nurse to join our Home Care team at ChristianaCare. As a valued member of our organization, you will provide skilled nursing care to geriatric patients in the community, ensuring high-quality treatment and support. Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Develop and implement goal-oriented, patient-centered care plans.
• Assist in patient discharge planning.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of start date.
• Current DE RN license or compact state license.
• Minimum 2 years of recent HomeHealth RN experience.
• Prior Home Healthcare and Med-Surgical experience preferred.
• IV proficiency or willingness to train and perform IV functions.
