Job Description

We are looking for a dedicated Registered Nurse (RN) Field Nurse specializing in Home Care to join our team at ChristianaCare. As a Full Time RN in our HomeHealth department, you will provide skilled nursing care and treatment to geriatric patients in the community. Working day shift hours with additional on-call responsibilities, you will develop and implement goal-oriented care plans, ensuring the best possible patient outcomes.

Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community.
• Develop and implement goal-oriented care plans, consistent with available resources.
• Plan for the discharge of patients.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of employment.
• Current DE RN license or compact state license.
• 2 years of HomeHealth RN experience within the last 12 months.
• Previous experience in Home Healthcare and Med-Surgical settings preferred.
• IV proficiency or willingness to train and perform IV functions.
