Job Description
We are looking for a dedicated Registered Nurse (RN) with experience in Home Health Care to join our team. If you are passionate about providing quality care to geriatric patients in the community and thrive in a supportive work environment, this opportunity is for you.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop and implement patient-centered care plans.
- Plan for patient discharge and continuity of care.
- Be proficient in IV functions or willing to undergo training.
Requirements
- BSN required; ADN candidates must obtain BSN within 3 years.
- Current DE RN license or compact state license.
- Minimum 2 years of Home Health RN experience within the last 12 months.
- Previous experience in Home Healthcare and Med-Surgical preferred.
