Paradigm Rehab is currently seeking a dedicated Rehabilitation Therapist to join our team at a competitive pay rate of $70-$90 per hour. We are looking for someone passionate about helping individuals reconnect with their environment to enhance their quality of life.

Responsibilities

• Conduct home visit Physical Therapy assessments and provide direct patient care.
• Assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation.
• Maintain CPR Certification.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Flexible scheduling options available: full-time, part-time, and per diem.
