Responsibilities

• Conduct home visit assessments of referred patients to assess medical conditions and potential for rehabilitation.
• Provide direct patient care and develop treatment plans.
• Document patient progress accurately in our EMR system.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficient in EMR documentation.
• CPR Certification.
